Kaiden from ME1 because I have it on PS3 and did some online comic where I killed him. 

I also can't keep Miranda alive due to her loyalty conflict with Jack.  Tried for high paragon and all that but still can't keep both of them alive.

I hear you can stagger miranda/jack with certain people during the suicide mission, but haven't bothered with that yet.
