I think it has more to do with the fact that a lot of people will abuse their access to the internet by looking at porn, and a lot of the times will do it at work. If you were looking a picture that had a penis in it, even though it's not porn, someone walking by will make the assumption that it is- and that can get you in trouble. Its dumb, but the whole NSFW thing is just a warning for people to cover their asses. If you're looking at a gory picture, you're still wasting time, but they'll just think you're looking up something about a slasher/action film, or something.
Blame the people that browse porn sites at work, and get nothing done or get their office's computer infected with a viruses for having to be cautious about NSFW pictures.
