The only one I actually killed without really realizing at the time how the dialogue options worked was Zaeed. I let him burn to death thinking it was going to give me an option to call his bluff (ME2 was the first one I played and I didn't know how paragon/renegade worked), and then again I didn't know how his loyalty mission worked, so when I redid the mission to keep him alive, I waited too long, paragoned it, and he died on the suicide mission.
Needless to say, this was not acceptable.
