each of the questions you posed would require a dissertation to answer them properly. They're good questions but it's difficult to answer them on a forum board. Google scholar is where you need to go.

Hey dude don't diss the disser's. They often have better ideas and answer than text books

substance vs art seesm to be high on the list of what makes a director great

You should read "Making Movies" by Sydney Lumet.

2. I imagine that answer varies from artist to artist across a whole host of mediums.
3. They identify with the content.
